Its eating him pretty much!

I have no clue what this is. If you cant see the link let me know. I don't know what it is. Its on my Red Sea Sailfin Tang. On his dorsal fin the disease or what ever it is, is making holes in his fins. I have never seen or heard of this please right back ASAP! Although he still is eating normally and everything is the same he hasn't been acting different he just has those weird spots on him for some reason.
How did he get it?
How do you get rid of it?

The history of the fish is... there pretty much is none besides hes like 3 years old. He has been fine up until this disease came along. And I haven't added a wet item in like 3 months
