That looks like a cloudy/ enlarged eye ... often the result of a bacterial infection. Eye infections can sometimes clear up on their own (fish's own immune system coming into play) .. but if you don't see any progress then placing the fish in a QT with some antibiotics may be waranted. A double dose of maracyn 2 should help.
