Leslies pool hardness plus need help…!

Hey guys I've been trying to find out on how to mix the hardness plus from leslies . I had already done in the past and it's been good so far. So now I'm running really low and will need to re-up and can't find the info on how to mix it. Can any one help me out on this. Thanks !
