LFS - Status Thread

I thought it would be a good idea to get an updated list of area LFS's and what the status is.. Open, Closed, Moved, etc.. I drove around to three that I knew of only to find two of them gone. I have since found out that one has moved, but it would be nice to know where to.. lol

This is not a sponsor list or a favorite lfs list, or a rate list or anything like that. Simply just an information list. How about it? Whats out there.

For simplicity lets keep a format, say something like this.
