Amphipods/Copepods. They are a good part of your tank and natural food for fish like Mandarins.

usually on live rock / coral frags / live sand

Absolutely the frags..
You get all sorts of life coming in on those..
worms, starfish, pods, etc...

Reportedly some of their eggs survive in frozen foods but I couldn't attest to this one way or another. Maybe just another urban legend of the fish world.
