Lost 5 fish in 2 days

I just recently lost 5 fish in two days. 40 gal breeder, tank mates 2 clown fish(still alive) 2 royal grammas(still alive) 1 mandarin(still alive). Fish lost 3 lyretail Anthias, Powder Blue Tang, Tailspot Blennie. Powder Blue Tang had him for almost 3 years recently had a case of Ick, he didn't show any signs of trouble, was still active and eating, and was not breathing heavily. The ick cleaned up or hatched he had some slight scarring and slime coat was not completely back yet but seemed like he was on the mend. No other fish in the tank have any signs of Ick!! The Anthias were new additions along with the mandarin got them from Live Aquaria, seemed fine and acclimated well. The Anthias were a little scared of the tang because he would zip around the tank. Tail spot blennie had for about 6 months was always active and seemed okay. All of a sudden the 3 Anthias, the Tang, hid behind the rocks for a day and then died and the Blenny died with in a day. All other inhabitants seem okay, all sps corals are fine no other losses. Anybody have any ideas what could have happened. Water parameters seem perfect. Please help put my mind to ease and help figure out what happened. Thank you all!!!!:confused:
 
Well, your tank is way too small for a PBT, cramped-caused stress could have contributed to ich outbreak. Same for the trio of anthias. Anthias are tough fish to keep and a trio needs lots of room. (LA suggests 125 minimum tank size for both the PBT and the trio of Anthias.) You simply cannot keep fish healthy long-term with ich in your tank. It doesn't just go away. You have a small tank and need to start thinking in terms of small numbers of small fish. Even your mandarin needs a big tank; to supply the live pods it needs to thrive. Your immediate problem is ich and you'll need a HT/QT. What is your current fish list? We'll go from there.
