Doesn't sound like velvet with only one fish affected.
Without pictures of the live fish in the water I can only guess but a bacterial infection is a possibility. I noticed that bacteria thrive under hyposaline conditions and some fish get bacterial infections easily there while others are totally unaffected.

Here are a few more pictures. Other fish are starting to look sick. I just had a yellow tang, maroon clownfish and a bi color Blenny die within two days. I'm not sure what's going on and all my levels are perfect on a 8 month system. I currently have a
90 gallon reef ready with refugium, protein skimmer, carbon and gfo reactor and a uv sterilizer. I added a blue tang that was already been quarantined from a local fish store directly to my tank. I think that's what caused all this.
