I just got a new MP40, my glass is 6/16 (at the most 7/16) so I'm using the 3/8 position on the pin spacer. Should I also be using the rubber spacer?

Yes, the rubber gasket should be used at all times.

The glass is exactly 10mm thick (.3937 in), is 3/8 the correct pin spacer choice?

That is correct, 3/8 will work perfectly.
