my anemone has heartworms.

Recently moved my carpet from his long term tank into a new 16 gallon. see pic below.... he looks better today, but not really sure what is going on here.

That does not look good, the foot must have a laceration or it is melting. The heartworms are the filaments that are usually inside the nem.
 
i think i tore him a little... i suspect that is what is happening here. Maybe EC can speculate on his chances of survival?
 
It's not his stomach. Those are mesenterial filaments. They are normally on the inside of anemones, but they are used as a line of defense in the advent of injury or attack (mesenterial filaments have stinging cells on them). The anemone has obviously been torn and/or is under attack by something in the tank. They can recover from this kind of an injury - as long as they don't pick up a bacterial infection.
