My Crocea not looking to good.

I bought a 2 inch Crocea about 2 months ago, after I got my new Nove Extreme Pro t-5 lights, on my 120 dt. He appeared to be doing very well up until yesterday, at which time I noticed that his mantle was somewhat withdrawn and the siphon hole appeared to be enlarged. Today, when I got home from work, he was laying on his side, and not attached to the rock, on which he had been attached to before. The siphon hole was much larger and only a small portion of the mantle came out over the shell edges.

My water conditions are as follows: sg-1.025, ph-8.0, ammon.-0, nitrite- 0, nitrate-10 to 20 (chaeto, not thriving in the refugium, as I would like). The only thing out of the ordinary is that the night before it started looking bad, I had supplemented the tank with Seachem, Reef Plus and Reef Trace. I also fed Roti-Rich. I hadn't supplemented any of the above for about a month, not sure if that is what has caused this, or what I should do to try and save the clam. Any advise?
 
Ok. I'll try and get a picture tonight. The tank is stocked with: Yellow Tang, pair of false percula clownfish, cleaner wrasse, 6 line wrasse, pair of psychadelic mandarins, coral beauty, blue and gold damsel, eyelash blenny. Over all it is a pretty peaceful tank. The damsel is very non aggressive. I would say that the yellow tang is the boss of the tank. I also have a large leather, some mushrooms, sand polyps, xenias, seabae anemone, and 2 rhodactis anemones. None of the coral are really close together. The clam was placed on a rock approx. 12 inches down from the lights. Since it stopped holding the rock, and layed on its side, I moved it down on the bottom in the stand, it still moves onto its side. It is somewhat reactive today, but definately isn't exposing as much of its mantle today.
