My LTA is dying!

I don't know what is wrong with my long tip anemone. This thing is gorgeous and huge normally. For some reason, he was uprooted this morning. There is no apparent injury from the power head and I see no reason to indicate that he had been sucked into it. This is what he looks like though, it's almost as if he's turning himself inside out. I don't know what to do. Any and all help is greatly appreciated. Thank you!

Need more info, tank specs/params/time running
They do ball up like that sometimes, but detaching and as it looks doesn't seem good.
 
1.025, 77°F, 8.0PH, 8dkH, 0 NH3, 0 No2, 0 No3, 0 PO4, 380ppm Calcium, 1100 ppm Magnesium, .05 ppm Iodine. No test for Strontium.

75 gal with 20 gal refugium. Phosphate reactor, UV sterilizer, skimmer, T5 lighting, carbon sock. Instant Ocean Reef Salts. Ecotech Marine MP40 powerhead.

Tank has been set up for a little under a year at this point. Anemone has been in the tank for about 8 months.
 
You could use some slight adjustments in params but nothing that should be an issue.
I keep seeing posts about bristle worms bothering sand dwellars, just a possibility, really hard to say though for sure.
Are you spot feeding?
You have a DSB?
 
the side the anemone hangs out on has a moderately deep sand bed....~1.5". The rest of the tank is only about .5". That's an interesting thought regarding the bristle worms and I'll keep an eye out. But it really looks like he's extroverted his insides....seems really odd. We have not spot fed him recently.
 
That's not nearly enough sand for an LTA -- you might as well be bare bottom. IMO. LTAs need at least 3.5 inches, the deeper the better.
