Mysterious Fish Death

I apologize if this post is not in the right area. I have a 25 gallon saltwater tank established for 6 months.

I recently went for 6 weeks without a water change, then did a 20% change. 1 week later my yellow tang (only in tank for 2 weeks prior died) AND my pseudochromis. I tested the water and Nitrates were 80ppm. everything else was 0. (salinity and temp were are same too) i removed the fish right away. and then changed another 20% water.

12 days later, I did a 20% water change, my hippo tang and percula were looking fine. I tested the water and Nitrates showed 40ppm. I also treated 10 aiptasia with aiptasia X.

Here I am 5 days later and the hippo and percula are now dead. I am devastated.

1) When the first 2 fish died at once, what should I have done?
2) How should i have recovered after 6 weeks with no change? (long story behind reason for delay, you don't want to know)
3) what do I do now?
 
Are you using RO/DI water?

Also, many water companies are starting to add chloramines to the water supplies, which are more toxic than chlorine to their water supply. You may want to call your water provider and see if they've started to add chloramines. There are special chloramine cartridges you can add to your RO/Di unit.
