Need advice on making QT

I have a 20g tank which is cycled with a little bit of LR and 2 inch bed of LS. I want to make it a QT since my 90g display has a clown with ICH on it. Should I leave the sand in the 20g QT or take it out? I plan to leave all fish in it for 4 weeks or so. If I take out the LS won't that make a ton of water changes and risk hurting them with ammonia spikes etc?

You won't be able to do any treatments in the 20 gallon tank with sand and rock in it that won't destroy that sand and rock. You are better off taking it all out and using an artificial, cycled, biological filter.
