Need help wiring icecap 660 ballast for 3 80 watt T5 bulbs

Awhile back I contacted icecap requesting information on how to wire my icap 660 for t5's they sent me a link with diagrams to wire 4 54 watt bulbs or 3 80 watt bulbs but the link no longer works. I need to know what equipment to purchase and how to wire it. If anyone has done this before and can provide assistance it would be appreciated.

Okay they are wired the same way I wired my vho's.
I just need to get the end caps and bulbs.
