Need help

So I have a 270 gallon aquarium. I have been battling some sort of disease for a year now and I finally pulled all my rock and caught the fish to put into quarantine for what I thought was ich. The tank has been fallow for month and a half. Im not sure what this disease is but I have a few fish that live with it I guess. They are the only ones who have not died. They are now in a 150 gallon rubbermaid tote and I treated them with cupramine for a month. I have a stars and stripes puffer who still has spots on his fins. So I treated Prazipro. Today I noticed that a regal tang has what appears to be pimples with some sort of grey parasite attached to him! ***. I have 2 Chromis, 1 regal tang, 1 stars and stripes puffer now. Can anyone tell me what this is?

I did a fresh water dip on him while the photo was being taken. I did not see any flukes come off. I do have formalin (quick cure) but wanted to use that as a last resort. I have not seen them scratching. I did lower the salinity down to hypo today. The puffer is definitely more active now. Back to his old self.
 
Me too on formalin. I want to use that as last resort . But have it available if needed. I use FW and seachem paraguard for external trematodes . I also have cupramine and chloroquine on hand for velvet and ick. Caution I don’t know if hypo is sufficient for externals . I would recommend on moving the fish to a new HT after dipping . The idea is to leave them behind and not reintroducing the fish back so as to get reinfected
