Odd fish behavior

My royal gramma is showing signs of change in behavior. He used to be very peaceful, sitting quietly, and then going for long glides across the tank. Now he darts in erradic movements, hides a lot, and seems to be constantly rubbing his face on rocks and such. He will sit still for a moment or two, then race forward, rub his face on a rock and change direction and zip away. I don't know why he's doing this, but could it be a disease?
Should I look for other symptoms of something?
 
It is trying to clear its gills but cannot........parasites I do believe.

The other cause I've read of is ammonia spikes causing irritation to the gills.
