One of my clowns died today, any ideas why? :(

Yesterday I noticed one of my smaller clowns looking faded instead of his normal bright orange. Today he was swimming around on his side and died this evening. He was still very faded.

I tested the water myself and the phosphates were at 11. Everything else was fine. I took a water sample to my LFS and they confirmed my results. I picked up some phosban for my FX5. I cleaned all my filters and put the phosban and some new chemi pure in.

Any ideas what killed my clown?
 
The test sample was pretty dark blue. He held it up to the color chart and I was 1 away from the highest. I just saw the number 11. I'm still too new know the proper units of measure.
 
+1 on the phosphates, thats pretty high,

Do you think it gradually went up or spiked out of no where?

You may want to keep some form of phosphate remover in your tank!

Sorry for you loss, loosing one of a pair is always a pain because you have to work on getting your pair again!
 
I test the water weekly and they have always been minimal. So my guess is they jumped in a week's time. I'm trying to figure out if I did anything differently.

I think the highest level was 12 on the chart.

The fish have been eating their frozen mysis pretty regularly.
