Parasite ID on yellow tang

This yellow tang has a single white protrusion at the very tip of its first dorsal spine. It almost looks like the tip of the spine sticking through the skin (but isn't).

Can anyone help with an ID on what this is? Is it a parasite? Or something else?

Tank parameters have been stable with no additions or changes in livestock for 3 months. All other inhabitants (fish, corals and inverts) appear normal and healthy. This tang is still eating and active. I'm just wondering what the best course of action here is.

Looking forward to your thoughts.
 
To clarify, tank is 110 reef with 7 chromis, 2 Ocellaris clowns, 1 blue eye tang, 1 lawnmower blenny and the aforementioned yellow tang. Several mushroom colonies, zoas, finger leather and green bubble tip anemone; various snails (cerith, nassarius), blue and red leg hermits, 2 brittle stars, 90 pounds of live rock with good coralline growth.

Water chemistry is SG = 1.025, pH=8.3, 20 gallon water change every 2-3 weeks. I run carbon and GFO.

The tank has been running for about 18 months now. All livestock was QTed 2 weeks before adding to display (QTed and added separately by species), along with dips for all corals and fish. I haven't had any issues with pests or undesirables, so whatever it is growing on the tang is concerning.
 
I can't tell if that's an ich spot or lympho. If it multiplies, its ich. If its lumpy, its lympho---aa harmless virus. Two weeks quarantine is not nearly enough; that isn't even one normal ich life cycle. , 6 weeks is adequate.
 
There's been no change. No more spots, no spread. Here are some clearer pics taken today:

Any more certainty that this is lympho?
