Could be some type of parasite although it's a huge one. Almost looks like he got stuck on a powerhead and now has a hickey. Any of your powerhead intakes not screened?
He has been stuck on a powerhead but this bump is getting larger daily. It used to have a worm looking thing hanging off of it and I treated him and it went away and then came back like this.

Could be a granuloma (particularly Mycobacterium, fish tuberculosis), tumor, hernia, or abscess. If it's Mycobacterium, it is potentially transmissible to humans ("fish finger" ) so wear gloves when you work in the tank. Conformation would involve a trip to the vet and ideal treatment involves antibiotic injections. It is likely the tank environment is contaminated with the organism and complete eradication would be very difficult. Empirical therapy with Kanamycin or Streptomycin in QT can be tried. In addition to the external bump you see, your puffer may have internal lesions as well (liver, kidney, ect) and a cure is unlikely.
Thank you gator for finally giving me somewhere to start. I don't like to sit and watch an animal die now I have somethings I can try and treat for and see if he gets better.
