Please help sick clown fish

Hi, we have a clown fish with white bubbles and seems to be white worn skin on his head hopefully you can see the pics. If there's anyone I can help please let me know I really appreciate it.

Ya I have the one it comes with and an extra fan, he's been with m over a year

Many fish will have problems that may never be seen untill its stressed. Low oxygen levels are very stressfull. Salt water recieves Oxygen from a gas exchange at the surface. Bringing in oxygen and releasing nitrogen. High nitrogen can also starve the oxygen.
This is done from good steady water flow. 15 times the tank size from 2 powerheads.
