IMO, formalin is bad for both you and your fish. I would suggest acriflavine (which the article talks about). Ruby reef rally is probably the easiest way to get it. Use it (diluted appropriately) as a bath, not as a tank treatment.
Sorry for the double post. So a damsel was added a month ago? They are susceptible to Uronema, which might explain how it got in your system. He is showing no symptoms?

I bough 4 damsels and 3 died within 3 days (about a month ago) so theres 1 left.
Unfortunately the fish just died an hour ago. I read about Uronema and it looks like my tank is done for it. The trigger may live but the other clown and damsel may ultimately die
Sorry to hear about your loss. If any of the other fish get sick I would dip in an acriflavine bath (Ruby reef rally for example) and treat both the tank with metroplex and feed metroplex+focus. Remember to watch your nitrogen cycle carefully if you decide to treat with metroplex, it can kill off bio filtration.
