1. How old is this aquarium?
~1.5 months

2. If less than six months old, what is ammonia level?
0

3. What is SG of this aquarium? How measured?
1.0125 using a hydrometeor. I should receive my refractometer shortly.

4. When was the last fish added to this aquarium?
1 week ago

5. Was it quarantined? If so, how? And how long? Was it prophylactically treated? How?
No :(

6. If you are using a copper based medication, which one? How often do you measure level? When?

7. If you are using hyposalinity, how did you calibrate your refractometer?

8. Please describe in detail, the appearance of the fish? If there is one or more pimples, are they lumpy? What color?
The fish swims and eats fine but one of it's pectoral fin is torn and appears to have irritation where the torn fin meets the body. This is very close to the gills. (The fish is a 6 line wrasse)

9. Please describe the behavior of the fish as best you can. Is it acting reclusive? Is it always up towards the top of the aquarium? Is it avoiding light? How active is the fish?
The fish is very active, swimming all around and inside of live rocks.

10. Is the fish eating? What?
Yes. Frozen Mysis and frozen brine

If I start QT/treatment for ich now, how long do I need to wait for my DT to be ok? The 6 line is the only fish in the DT, so it would be fish-less.

I plan to use the tank transfer method...
 
Hard to tell for sure from the pictures, but there appears to be white spots or bumps on the left side of the wrasse's body. Could be flukes or crypt. I would pull the fish out and first start with a round of Prazipro to see if that resolves the problem. If it does, you can treat the DT with Prazi (just don't run the skimmer or carbon). If that doesn't resolve the problem, you'll want to treat for crypt using one of the recommended methods (tank transfer, copper or CP, hypo). You'll need to leave your DT fishless for 10-12 weeks if it is crypt.

Thank you for your reply. I am moving the fish to QT once it has been trapped.

I have Prazi on hand, but I do not have an established QT. I would like any advice/opinions for the next step.

Do you think Prazi combined with the tank transfer method would be too much stress? The directions on my bottle of Prazi say 5-7 days for treatment. So I could use it during the first 2 transfers and see how that goes...

I personally would administer one treatment at a time. Prazi first, then tank transfer (if needed). If it's flukes, you don't have to go through the added stress of TTM. If the fish is eating and otherwise acting normal, you have time.
 
Instant Ocean bio spira works great for quickly establishing bio filtration.

I thought I had read people advising against using products like this, but after doing more research it seems many people have had great success using this. Thank you for the suggestion.

My trap failed to catch the 6 line today; however the fish looks much better today. I still plan to QT though.
