puffer blind......need help

this is my second puffer to go blind.

my parameters:
specific gravity: .25
Ammonia: 0
nitrite:0
nitrate: 25
PH: 8.2

He is in a 40g QT tank with about 30lbs of LR and a Coralife Superskimmer 125.

I bought him 3 days ago and have not been able to get him to eat. He is a juvenile.

any suggestions?

His eyes are covered in a white film. He also appears to be breathing heavily.
