Puffer not eating for over a week! Any advice?

Hi, I have a 55 gallon FO setup that I've had for close to 20 yrs. Currently I have 1 green spotted puffer and 1 yellow tail damsel. Until recently I also had 1 very large domino damsel and 2 three striped damsels.

Long story short about 2 1/2 yrs ago I went away and had a friend watch my tank, the heater stopped working and all of my fish died. I wasn't sure if I was going to keep or break down the tank so I just grabbed a few damsels and never returned them. I got the puffer a few months later, all was fine until about 6 weeks ago.

The domino became very aggressive picking on everyone and about 4 weeks ago I re-homed him. This is when I started noticing changes in my puffer. He started hiding more and eating less and his color changed to dark most of the time. After the domino was gone the puffer bounced back and was eating and swimming fine. I treated the tank with Melafix for a week to heal everyones fins, etc. about then the three stripes started becoming aggressive and circling the puffer all the time. The puffer turned dark again and spent most of the time hiding in a cave or laying curled up somewhere on the bottom and would barley eat. Needless to say they were re-homed last week. I did a major WC and rearranged the tank to give him some more spaces to explore/ hide.

I can't get him to eat. In the last week he has only eaten a few small freshwater snails. I have tried clams, scallop, shrimp (al from the grocery store) mysis, krill, live ghost shrimp, hermit crabs, snails, black worms. He's interested in them but doesn't eat. He is still dark mostly and lays around and hides. I don't know what to do.

Water quality is good, no ammonia or nitrites, I have nitrates (about 40) but they are less than they were about a month ago, pH is 7.8 and salinity is 1.020 I'll be doing another large WC today.

Puffers can be very finicky. I have a porc in QT that suddenly went on about a week long hunger-strike as well. Since these are predatory fish I think it's normal for them. He started eating a little over a week in and has been back to eating daily ever since. I wouldn't worry too much unless you are approaching 10 days+. Also, puffers can get to where their teeth are too long and it interferes with their ability to eat - have you looked at his teeth?
 
Thank you. His teeth appear to be fine. I'm nervous because when I had a porc when it acted like this, he died soon thereafter. And I have to go out of town at the end of the week for a few days so I'd really like for him to eat something before I leave.

I got mine to start eating again with a littleneck clam. I cracked the shell open and put it in the bottom of the QT overnight (puffers are nocturnal). He didn't eat it violently or anything but he did pick at it quite a bit. The next day he was eating my usual homemade frozen cubes.
 
I'll try that tonight. I clipped a shrimp to the glass the night before last and nothing. I put a oyster on the half shell in there one day last week also. I really thought the live ghost shrimp would have done the trick. He was interested but didn't eat them. Although I couldn't find some in the morning. I just assumed that the died and got sucked into the filter. Maybe the clam will work! Also on a gross note, I saw him poop two days ago and it was white, not stringy but white. It's usually reddish-brown. I figure it's because he's not eating that much if anything. Could he have developed a parasite after 2+ yrs with no new inhabitants? I did put a small piece of live rock (my first) in there last Thursday......ugh!

I'm literally losing sleep over this. He's got such a nice personality. Sometimes when I'm not looking he swims around and does his usual flips, and while he still has a dark belly, his spots are there. I'll give you an update after the clam. Thanks again.

Could be an internal parasite or worm. I would pickup some Prazipro and dose as directed, do a 25% water change and re-dose a week later. It would be better if he was eating first since Prazipro can cause temporary appetite suppression, however if he has a worm he's not going to eat anyways. I hope he comes around for you.
 
I just got back from my trip and puffer is still hanging on. He ate about 10 small FW snails on Wednesday before I left but as far as I can see, nothing since then. I tried the little neck clam the night before I left, but he didn't touch it.

I picked up PraziPro while I was away since no place around hee had it. Doing a WC tonight and adding it. I also picked up clams on the half shell (tiny clams that they feed to puffers for a treat I guess) just put one in and so far, nothing. Keeping my fingers crossed!

It's good that he was eating the snails. Mine was picky for a day or so after the prazi dosing but started back up after a day or two with each dose. Did he at least pick at the clams? I left mine in overnight and while he didn't devour them he did eat. It can be so frustrating but patience is key.
 
He wouldn't touch the clams and I did leave them in over night. Last night he was bobbing pitiful at the top of the tank where I usually feed him, very dark in color. I had the frozen clam strip fish food that I was trying to feed him. I got mad and literally shoved it in his mouth and he ate about half of it, so at least he won't starve hopefully. I currently have calamari from the grocery store soaked in garlic guard clipped to the side of the tank and another one hanging on a string and I'm stalking him while I type right now. Nothing yet. I even smashed one of my SW snails but he didn't touch it. I'll be getting more FW snails tomorrow (only a 4 hour round trip!) I'll keep you updated! Thank you for all of your help!
 
He ate ghost shrimp tonight and possibly a few more snails. I'm feeling a bit more optimistic tonight! ������
 
So fast forward to 2 weeks later and I am still having issues. The last time he ate was almost 2 weeks ago. I dosed with 2 cycles of PraziPro and did a 20 gallon WC on Saturday. Today he attempted to eat a piece of raw shrimp but didn't actually bite it. My concern is that his poop still looks white and stringy (see attached photo) but he went through 12 days of PraziPro. He was miserable when the PraziPro was in the tank. He laid on the bottom in the corner and was very dark and breathed heavy. I added an air stone and powerhead.

I'm concerned about his teeth but as you can see they aren't protruding or anything. It's just the day that I shoved the clam strip in his mouth I noticed when he was chewing that they were pointy. There is no one nearby that can assist me with trimming his teeth and I don't think he'll live through the sedation.

I am out of ideas. Any suggestions would be greatly appreciated.
