Did you see any other odd behavior before death? Fish swimming into the stream of a powerhead is a common behavioral sign of velvet. Along with heavy breathing but that's not unique to velvet. Velvet normally kills quicker than this but your formalin dips may have bought it some extra time.

I did. I saw heavy breathing, loss of appetite, shyness, and lethargic actions. I wanted to help but couldn't ascertain what my next steps should have been.
