QT tank making my fish sick..

Hey guys..so i recently had an ich outbreack got everything under control and finally am getting back to normal so i picked up a new lionfish..i put him in my QT to start his treatment and it seemed right away he got cloudy eye now hes breathing somewhat hard..my water is completely fine in the QT im wondering if a sickness from another fish carried over and got him sick? He is on a treatment right now but doesnt seem to be getting better...any ideas? From my experience a QT tank has only done bad for me so far and im tired of losing fish since i lost my Achilles and Naso due to ich any help is much appreciated!

After each successful round of quarantine, sterilize all the QT equipment. Place all filters (no foam filters), pumps, nets etc, into the aquarium will a cup or two bleach for 24 hours then rinse all the equip. I'll even wipe down the outside of the aquarium and work station with a towel dipped in the bleach bath.
