I would ditch the stone and get a long bubble wand. They might be breathing heavy because of lack of available oxygen in the water.
If your having ammonia issues while doing TTM, your feeding them way too much. I've never had an issue with ammonia during TTM. The fish aren't in the same water long enough to cause an issue with ammonia. I've used the seachem ammonia badges before and never saw them move. I don't even check for it now while doing TTM. I will admit that I've only ever done small fish, so it may be different with larger ones.
I've had fish never eat during the entire TTM process, but as soon as they go into a QT, they usually start eating. I wouldn't sweat it.
Fish dying unfortunately happens, and often we never know why. I doubt it was the TTM process that caused it. A lot of people have never had a fish die during this, and its actually the least stressful of the three methods.
It was probably unhealthy to begin with. This is why we put these animals through this(TTM, QT procedure), weed out the unhealthy before adding them to our DT.
