Red spot on clown fin

Yesterday I noticed that my clown has a red spot on his front fin. It's round and totally flat, about 1-3 mm in diameter. He's eating well, but sometimes thrashes around in the sand for a second or two, which is new behavior for him. I've had him for several years and there have been no new additions to the tank in at least a year except for some new aragonite (dry) and some feather caulerpa a few weeks ago.

His only tankmates are a yellow tang, a micro hermit, and some live rock. No corals.

Nitrates were high...about 90...so I did a water change. Ammonia and nitrites are 0. What else should I do? I don't know if it's an injury, a parasite, or an infection.
 
I would just watch the clown for now. It is impossible to diagnose at this point. You might want to have antibiotics on hand, such as Furan-2 or Maracyn Plus.
