Royal Gamma - Help in identifying if ill

Here is the background on this guy -

I had a 36 gallon tank that I added the gamma, and a blue damsel to back in June. They were the only two fish. A day after I added them to the tank I noticed white spots that appeared to be ich. I removed them from the DT, and put them into an HT. There I did a hyposalinity routine 10 weeks from start to finish. Dropped salinity down to 1.010 had the fish in hypo for 5 weeks (never saw any ich cysts after I moved them from the DT)... Brought the salinity back up. Then did another 4 weeks observation.

During the total 10 weeks the tank was fallow. No other fish, just one hermit and some snails. No feeding at all during the entire period as well, and lights were off the whole time.

I added the fish back after the 10 weeks, in June and there have been no signs of ich. Fast forward to September and I upgraded to a 100 gallon set up. Used the old rock - added all new sand and another 60 lbs of dry rock. Since the move, both the damsel and gamma have been doing fine.

The gamma has always been VERY shy, he almost never swims around freely and usually hangs out under and behind things. He eats fine, especially when I feed them mysis shrimp.

I've recently become concerned when he was in hiding up close to the tank and I got a good look at him. I got an okay picture that kind of shows why I'm concerned. His tail fin appears to be slightly tattered, and he has a red line on his face around his eye. I don't know what I could be an would really appreciate any help.

I currently have 4 new fish in QT (foxface, yellow eye kole tang, and 2 clowns) that I want to add to my DT after I finish up there cupramine treatment in a couple of weeks. I really don't want to add them if there is any chance this fish might be I'll.

Thanks for any input, it could be nothing as I've read it's possible for their fins to get beat up from hiding in rocks (which he does a lot).

You can kind of see the red line that starts at his lip, goes through his eye, then continues past his eye. And, I just looked and the lone is only on one side.
 
Thank you!! I didn't get any sort of notification that you replied - so sorry for the delay. I also noticed he's looking quite skinny, he's very secretive and hardly ventures out to eat. Hopefully he will be OK, I've at least ruled out any sort of illness at this point.
