Sailfin Tang Disease?

Are the holes on the side of the head normal or a disease? I see a lateral line in the still shot.

I've been reading that water quality and diet are the main causes of HLLE. I feed my tang a finger sized piece of nore every other day. He eats it all within about 15 minutes, should I feed more? I have a reef tank with GFO and Carbon reactor and protein skimmer. Any issues with a ULNS tank?

I also read that stray voltage can cause it. I use grounding probes in the display as well as the sump, but is there a way to test to see if there is voltage in the tank? How much is bad?
 
It is HLLE and very common in tangs, no one knows the cause but it has been blamed on everything

How big is that tang? My PBT is 4 inches now, and even when she was the only nori eater in the tank she would get a 1" X 2" sheet of it every other day.
 
Its about 4-5 inches. I guess I could increase the nori sheet size until I see it not gone when I get home from work.
