sand sifting starfish dead

Can home last night and my star fish was on the bottiom of the tank with one leg in cut in half and lifeless...Water test showed all the parameters OK. Only thing I have done different is add iodine as required for a 130 gal reef and fish tank. Did a 1/3 wat4er change this past Friday.

Does anyone have any idea what may be going on

Does that mean that you add iodine but don't test for it?

It might have starved. How long has it been in the tank? Is the tank only 6 months old? This seems to be a difficult animal to keep alive for long.
