sick clown help

Have a clown living in a sabae and his eyes look puffy and it seems he can not see very well and won't eat,

Sonds like a bacterial infection. Remove to quarantine and treat with antibiotics.

He is still alive, but stays in the sabae, and he can't see, his color is changing on his yellow stripes, I have had him for about 5 mounths.

Not sure what issue your fish has ... eye injury/illness is often a bacterial issue which often can be dealt with with nothing other than good food/good water quality (fish's own immune system handles it) .. if its a bacterial issue and the "chicken soup" method doesnt work then QT combined with an antibiotic may be necessary (maracyn 2 is readily available and should work with this type of problem).

However .. if film covers more than the eye then you should consider Brooklynella (clownfish disease) which is normally treated with formalin.
