sick/dying open brain coral

I have an open brain coral that seams to be dying. The skeleton is showing and I cant get it to take any food. The sweeper tentacles used to come out at night, but now it does nothing. I took it out the other day and gave it a dip in Coral rx, and got a couple of bristle worms that came out. Parameters are all within spec. Its in a low light/flow part of my tank and nothing seems to be bothering it. I'm at a loss and don't want this guy to die. I'm on a work computer so i cant post pics, but i will try to load one later in the day.

Is it bleaching or does it still have color? How long has it been in your tank? What livestock is also in the tank?

Generally, LPS like some nitrate. I see your's test at zero. What test kit are you using for nitrate?
 
I have mostly LPS in the tank. Two clowns and a colony of zoas. All my other corals are doing great. I skipped my water change this week in hopes to get the nitrates up. I use red sea's test kits. I'm gonna be moving it to another tank by itself tonight. I plan to feed in zooplankton and try to nurse it back to health. Seems to be bleaching so i built a cave for it in the other tank. I got some advice from a lfs that I trust so I'll keep you posted.
