sick grouper

I have a neighbor with a sick grouper and I told him I would but this on the board and see if we have any idea what this is....

It is raised and almost looks like a scab....

P.S. It's been at least a week and it stopped eating about
5 days ago...

This fish has a bad bacterial infection. He should be removed to a sick tank and treated with Furan2. Hopefully you can save him. He needs to be treated immediately.
