The closest match I could find was Astreopora listeri.

It doesn't say anything about the polyp length, but the skeleton and overall appearance seem to match. It may just be that this species is less frequently imported than the pink/purple with dome-shaped corallites Astreopora that you typically see. You could ask Eric Borneman at his forum over on the Marine Depot site.
