slowly withering away

I purchased some frags from a LFS and this one appears to be dying or something is eating it. Any Suggestions?

Nitrite 0 ppm
Nitrate 5 ppm

I think something is bothering it but who knows what it is? you should do some investigating.

I have had it for about two weeks.
Also the SG is 1.0245

First I would id it, then research the lighting needs ,and the flow requirerments. Alot of times these are the two issues that I find affect our corals if all parms are in check.
