small Maxima clam just died

I'm not sure why but my small maxima clam just died last night so I thought I'd post a thread to give other people information that might help them avoid this happening to them. I will check all of my levels a little later today and post my results. At the moment I was just checking on the tank in the dark because I woke up and couldnt get back to sleep.

The clam was only about 2" and I have had it for just over a year. I bought it from a friend when his tank crashed in a fire started by one of his lighting ballests last summer (only his clams survived and they survived for 2 hours out of water). I kept the clam in my sandbed under 2 250W MH in my 120 gal with an ASM G3 skimmer and a 20 gal fuge with DSB in the fuge. You can see a new pic of my tank in my gallery (the pic is actually from august not september). I also still have 2 other clams: a larger 3" maxima and a 3" squamosa.

In the last week the maxima that died had been sitting next to the squamosa in the sand and kept falling over. For a few weeks the clam had just sat in the sand but in the last 3 days I had stuck a rock underneath him because my clown and engineer goby move the sand around a lot. This past week it looked like it was showing signs of pinched mantle. I should have taken a picture: it was mostly closed up except that the mantle still stuck out of the shell a bit and mostly just stuck out straight rather than foulded over the sides. It would also constantly fall onto its side and would neither anchor into the sand or attach to the rock for very long. Something else worth mensioning was that this hasnt been the first time it has looked this way so I just figured that it would correct itself if I left it alone. I knew the clam was stressed for some reason because it was shooting off sperm wednesday night which I had never seen it do before. However the only change I had made to my tank in the past two weeks was to add a hammer coral.

I did my weekly 15 gal waterchange yesterday and checked a few things.
My Ca was 400ppm
my temp stays around 78 to 82 mostly at 80
my salinity is 1.025

I will post more information a little later as I'd like to get some more sleep right now.
