Something is picking at new clams mantle

I've had this clam for 2 weeks now and the poor thing is doing good considering what it's been through. I don't know how much more it can take. I first placed it in the sand next to a large piece of LR directly under 10k 150w mh. Every day it would end up on its side so I would prop it back up only to have the same thing happen again the next day. 4 days later I moved it up on top of the rock where it fit snugly but could still open all the way up. I came home from work and something had pushed it off the rock and once again it's on its side on the sand.

This went on for 3 days and I did catch my 5" lawn mower blenny knocking it off the rock. The blenny sleeps in a cave inside of the rock so hes pretty territorial when it comes to the rock but I didn't think a clam would matter to him but it does. Sooo, I moved the clam away from his rock more towards the middle of the tank but still under the mh bulb.

2 days later the clam seems to be doing fine as he is fully extened and color is coming back to it's original dark blue that it was in the lfs. I came home from work on the 3rd day to find that something has chewed a hole/gash in it's mantle. The cut is 1/8" long and 1/16th " wide right on the outside edge of the mantle. The clam is still fully extended and looks great except for the tear.

This morning I covered the clam with a plastic strawberry container so it will be safe until I can find the culprit. The no.1 suspect has to be the lawn mower blenny but i'm not ruling out any of the other fish in the tank. Can someone please tell me if any of these fish are known clam pickers.

no.2 suspect is coral beauty angel
2 percula clowns
yellow tang
royal gramma
2 yellow 3 stripe chromis
cleaner shrimp who stays in corner on other side of tank
turbo snails
hermit crabs

the blenny and the coral beauty.
yes the tear will heal itself naturally, but takes a little longer than fish heal fins.
