Tailspot blenny death.

So I can home to find my tail spot blenny completely verticle in the water. I thought somthing was wrong so I went to check him out and he had fallen to the bottom of the tank. And he turned white. And I noticed he was breathing heavily and for a split second has tiny almost white or cream bubbles come out of his mouth. So I called my LFS and they said to do a freshwater dip and I did that. I heated up water to the same temp of my tank and put him in there stirring my finger around to keep water goimg through his gills. He started moving and freaking out so put him back in a little bin ful of saltwater then when he calmed down I put him back in freshwater because that seemed to make him "alive again" but he slowely started loosing it and all if his fins were twitching while his body was completely still then he wasent breathing anymore and he died. His back find had 1 part of it missing but not bad at all. He didn't have white spots or anything like that. But he had 1 black dot close to his head and his eye and underneath he had weird tan metallic looking flesh or scales I'm not sure and looked to have an indent where those were. He wasn't skinny or anything like that. He almost looked bloated just a tad. I haven't had time to test my waters after he died but I'm doing that Tom but last time I tested my waters 2 weeks ago they were:
-nitrites 0
-nitrates 20-40
-ammonias 0
-salinity 1.024
-kh 8
Ph- I'm not 100% shure

Not too sure base on your description, it could be velvet or anything like nitrate poisoning. If you tested your water 2 weeks ago and nitrate was 20-40 which 20 is not too bad, but at 40 is high and would hate to see what the level for nitrate is now 2 weeks later..

I would test your water just to see how high it is and do a water change... Keep an eye on other fish.

Also when a fish is in badly poor state and weaken it is not a good ideal to put them through a fresh water dip or bath as it can be hard on them. If it was velvet fresh water would have done nothing as fresh water does not cure that.
 
I would agree with u but they have always been that high. And I'm keeping a rise bubble top anenome, urchin, sps and all of those are perfectly fine:/
