Tang Diagnosis

Hello,
My Kole Tang has been in quarantine for almost 1 week. He has deteriorated and crumped on me. Today he had labored respirations and responded intermittantly to stimuli. One of his eyes was Opaque and he had erythema on the base of his pectorals and underside. He was in quarantine with a blue tang. The blue tang looks great; is eating well and shares no similar manifestations. No fighting occurred. My Ammonia levels are non detectable. Any diagnosis?

The fish hadn't been scratching himself on LR. I had the fish for almost 1week in a bare quarantine tank (there was some PVC pieces in the tank). I purchased the fish from a local seller that had the fish in a bare tank. I'm not sure where the seller purchased the tang.
 
The fish's behavior? 1st day grazed on algae, 2nd day ate flake and then nothing. The fish was very shy. It mostly hid in the PVC. It would stay in close proximity with a smaller blue tang (same PVC price). I saw no fighting.
