Personally, I think it will be fine, size-wise. The whitecheek is among the smaller of the acan family, and I think would be fine in a 4' 120g. Disease-wise, I've read they are not as disease-prone as the other tangs, but that's really relative considering all tangs are quite disease-prone compared to other families of fish.
 
Personally, I would like at least a 6' tank. Years ago I had one ( Acanthurus nigricans ) in my 75 ( also 4' long ) and it just wasn't big enough for it. Before I sold it it was starting to pace back and forth, and managed to go from one end of the tank to the other in about 2 seconds.
 
The propensity for disease really has no relevance on the size of the display tank (unless it's grossly undersized). It bears relevance on the difficulties in getting the fish healthy (In QT) to go into the display tank.

From what I have read they are much hardier than Powder Blues and Achillies. But I haven't owned one myself. I do have a Powder Blue myself in a 90 Gallon that does great. Never had any disease problems at all with him and he eats like a pig.

A 120 is fine for the powder browns I would say. Just cater the rock work to the habits of the fish. In this case offer lots of swimming space.
 
well I dont think either are disease prone... maybe a bit more inclined to have parasites, but not diseases. it should be alright if it is one of the only larger fish in the tank, though you're better off with a powder brown (A. japonicus) over the whitecheek (A. nigricans).
