I assume this only works if the fish has not been "infected" by Ich Correct?

If this is the case how can one be sure the fish has not been infected (white spots not large enough for naked eye to see or in the gills?

No, if a fish is not infected the TTM would do nothing. What the method does it outrun the parasite's life cycle. The whole reason you use it as a quarantine method is because you can't always tell a fish is infected. After TTM the fish is then ich free and can't transfer the parasite into the DT.
