It's hard to tell from one picture. Sixline wrasses produce and sleep in a mucous cocoon, so you could be seeing the remainder from that.
Your tank looks new; how long has it been set up? How long have you had the wrasse? How is it acting (sixlines are curious fish, and should hunt and peck throughout the rockwork all day long).

He's doing great... like 2 weeks.. old wrasse. No abnormal behavior. He is also eating. I think it was just something got stuck to him. Is gone now. And my tank is pretty new about 5 months old... i dont keep the lights on much. That might slow down the tank process?

Looks like you have him in QT. Combining Seachem Metroplex + Kanaplex would cover both possibilities.
