Time to move it

I have a WBTA whose size is diminishing. I want to move itr to my nano tank to try and nurse it. I have no idea what it wrong with it, my other anemones are doing well, in fact my RBTA just split 4 days ago...quite cool it is the first one that split on me... I feel like such a good reef keeper! What is the best way to move this little sick anemone? I certainly do not want to make it worse. Suggestions anyone?

The W in Bta does that stand for white??? If so then yep that poor little guy needs help
Can you just pull the rock out that it is on and acclimitize it to the new tank and then in it goes?
If not can you get a fingernail under the foot and very carefull just loosen the foot off?

the rock it is on is to big for my nano. I will try to dislodge with my thumbnail. Thanks
