torch head dying why?

Why is one my torch heads dying? was doing great for the last 5 months

led lighting = 126 w 14 rb 14 cw mixed string of 6 blue,4 uv, 4 green. also running bio pellets,cheato in sump , algae scrubber and carbon.

all other coral seem to be doing well. thanks for the help guys.
