tunze 6060 broke but still under warranty

Hey my turbelle stream 6060 broke and i bought it in august of 2004 so it is still under warranty, I was wondering if you could give me the address to send it to for repair, and any return number or just tell me what I would have to put in the package and if to put a note in it or not and what to write on the box.

It is warranty if it is not the result of any of the following- use on a wavemaker, impact with a snail or crab that caused the shaft to break or running dry. Please include all of your contact info in the box.
