What are reasons for major tank crashes?

Just starting a new tank. Would like to, of course, prevent any future tank crashes. What are some reasons why tanks crash? And how can it be avoided?

Prolonged power outage and equipment failure.

You can buy 2 heaters of less power. If one jams it won't be strong enough to boil the tank. The other one will just turn off. If both jam that is just some crappy luck.

ATO's with float valves, sumps with float valves and DT's with float valves connected to kalkwasser mix. Don't do it.
