oh man, it could be any of a number of stresses. How long have you had it? Did you recently move it? Too much light, too low/high of salinity or calcium, stings, poor water quality, shipping stress, not enough food, etc.... ~Charles~
The euphyllia family that the hammer, frogspawn, and torch belong to are VERY sensitive to sudden light changes. They need to be acclimated VERY slowly under brighter lighting conditions with great care, and with a general regard under less than Metal Halide intensities.
If lighting isn't a factor and you are sure of it, stress from other chemical warfare or a sudden change in water chemistry can have a simmilar effect. Treat it as a bleached anenome and start target feeding it regularly, in a few weeks/months it will recover, but slowly.
