I think it might be mesenterial filaments.
I have seen them come out in various places in the flesh on my echino when I target feed it.
Does it retract back into the body? If it does, its normal I think.
But if its out all the time and getting worse, then it could be something else.

They do retract and come back from day to day. They are not like the tenticles that feed, but more like "spagetti shaped things". Is this healthy? What causes it?

They are mesenteries. They are the digestive organs. Mine does the same thing from time to time for no apparent reason and without any ill effects. As long as it's not gaping or melting off the skeleton (yours doesn't seem to be) just leave it be.
