What is wrong with my blue tang?

I know the tang has ich, but is this ich or something else. I have started feeding with selcon, any other advice.

Any closer shots?? Looks like it got stuck on a small power head. Is there anything in your tank that could have stung it?

i cant get a closer shot without zooming in on the picture. it looks like he scratched it raw, it sort of protrudes out from his body. He has ich real bad, but cant really tell in the picture.

What else is it doing? is it breathing heavily? acting like it wants to scratch it off on the rock work, substrate?

Try feeding some broccoli as well? Looks as though your little tang possibly has Hexmita/Spiro??(spelling) "hole in the head" disease.
This could cause loss of tissue and infections. What are your water conditions and how are you on water changes?? Be honest now

Water parameter are in check, i do monthly 30 gallon water changes. The blue tang has been doing great for months, then i added a new chevron tang(also has ich) about three weeks ago and the blue tang has went downhill the last 3 days. They both eat like pigs, lets hope they are healthy enough to pull through. The chevron does not look bad, but the blue looks real bad on one side. All other fish show no visual signs of ich. I think my tomato clown is harrassing him causing it to stress. I have not seen it scratch, but that could be the problem.

Check at night time for isopods, you have to be quick or else they will see you. The look like rolly pollys from hell.

Other than that it looks like the fish has some HLLE.
(Head and Lateral Line Erosion.)
The number one cause believed to be improper nutrition.

I have never seen HLLE in such a condensed area.
The gash just looks like something beat the brakes off it.

Feed it a good varied diet and reduce all stress in the tank.

175 bowfront with 30 gallon sump and only 1 - 30gallon water change a month is on the low side. Salt is "cheap" I would do more water changes.
